I hope that it wasn't caused by something that the insurance won't cover.

Thinking of problems caused by external contractors, who may or may not have liability insurance.

Pulled a cleat out of the fuel dock ? :D I've always been amazed at the space cadets that leave the boat in gear held with a spring line run to a cleat that I wouldn't tie my dingy up to. And they just wander off and go buy beer while the boat gets fueled.
 
Footage has surfaced of the Port Angeles mishap.
 
What a horrible accident! Lucky no one was hurt.
 
Accidents are always something nobody wants to happen. I am very sorry when the collision of these two boats happened.
Perhaps each of us needs to improve and pay more attention to avoid accidents. Unnecessary troubles like this
 
Over the years I've found that every single "incident" that I've had on my boat could be traced back to two specific issues.
1. My lack of attention. I got careless or over confident, especially in regards to electronic aids like autopilots.
2. Poor maintenance. Manufacturers specify replacement intervals on many "consumables" on boats. Things like filters, belts, and hoses. Changing oil is pretty straight forward but what about things like the bellows on a PSS? The manufacturer recommends replacement after a few years but people I've asked are generally unaware. Or a poorly tuned engine that stalls when you're docking?

We're all guilty of this to one degree or another. I'll post a link to an AOPA video. If you have the time spend a few minutes and learn from this pilots mistake. Airplanes are generally less forgiving than boats but the principles are the same.

Good video, sad ending. In my boat build, I asked what would happen if a certain system failed at sea and did not like the answer; so we enhanced that system with a backup of sorts.

MIA, This is so ironic, I watched this episode just the other day.

It is a small virtual world, I watched that one the other day too.
Pilot had to override his automatic eject that would have followed the first one!
Gotta be careful when the pilot yells "Hang-on" to just what you hang onto!

Read that too. Seems to me there should be something in the pre-flight brief that says "do not pull this lever" Must have been a good pilot to keep his own seat from ejecting!

Blue Bell, that's one of my favorite quotes. A cousin who was an Air Force fighter pilot and test pilot told me that way back in the 60's. Of course he went on to work for TWA and lived to a ripe old age thus disproving that saying because he was a really "bold" pilot if any of his exploits were to be believed.
