Last voyage for Costa Concordia cruise ship

    There is a whole chain of responsibilities, complicities and complacencies involved in this accident, it is more than obvious. It goes from Shettino (obviously), to Costa Crociere top officials who now claim that those pass-byes were not authorized (but the evidence says that it has been done more than 50 times in the past), to the major of Giglio who was so happy that Costa ships were paying the honor to his island, to the Coast Guard, who now play tough guys through the figure of De Falco, but has done nothing in the past to stop this tradition, etc, etc, etc.
    Schettino is at the wall because the ship was under his command (he is officially "comandante Schettino" - which mighty De Falco from Livorno has abbreviated in just "Shettino", to show who's who there) and because he is weak by nature and due to his conduct.
    Costa Crociere is too powerful and important society in these hard economical times, gives work to almost 20.000 persons, so don't expect them to pay for anything that has happened.
    Coast Guard has found it's sponsor in the news media, thanks to De Falco's yell "vada a bordo, cazzo", and is currently the heroes of the situation.
    The major of Giglio is not involved in the route planning of Costa Ships, so he is not liable for the disaster, though he could have complain to the authorities if he thought that those close passages are unsafe.
    So, at the end... Schettino (and perhaps his officers) will likely pay for all of them.

    Its not so important to PUNISH people. Far better to improve the system and make it safer. Italy needs better trafic control, VTS ship reporting. Now is a good time to invest and make jobs.

    Any case, if you have 4000 people to think about and instruments cannot help you, keep your ship in safe waters!
    Today is officially revealed that the idiot had brought with him a clandestine Romanian who was on the bridge at the moment of collision.
    A ***** was missing in this story disgusting....
    about other responsibilities, the second and third commander landed with Schettino ....
    The command of the Concordia was taken by a commander of the Costa, casually on Concordia as a passenger.

    A mental hospital !!!!

    I have worked in an office where ISM is practiced/accredited.

    It is similar to ISO Quality Management System (or any other quality management system like Six Sigma, 5S, ect.) wherein written guidelines are followed, formulated, and audited regularly. Just like the ISO, it does not guarantee that quality management will a guarantee a quality product, and ISM will not guarantee a safe ship. It is the management that formulates their procedures, form the organization, and assign personnels to do the task/responsibilities in accordance with the guidelines. The system works because the management tries their best effort to improve their system by spotting mistakes (non conformance in their terms) ,PREVENTING mistakes, and introducing policies to perfect its system. Complacency on the part of the management will result to losing its ISM accreditation if external audit find violations in the procedural manual that the accredited organization has written.

    True, the captains are like "little presidents" on a "mobile state" but basically they are line managers reporting to their land based based superiors which could be an operations manager, a general manager, or the ship owner himself. If you board a modern ship bridge and get past the steering and ship control areas, you get an impression of an office with rows and rows of maritime books, (SOLAS, MARPOL, ect), documents, company policies, procedures, besides the computer, laptops, and other direct line communication to the land office. The captain is just a link operating remotely from the conglomerate that controls him. But just like a link, it may break and brings the whole organization down with him.

    Below is what ISM is about.

    The ISM Code provides an International standard for the safe management and operation of ships and for pollution prevention.

    The purpose of ISM Code is:
     To ensure Safety at Sea
     To prevent human injury or loss of life
     To avoid damage to the environment and to the ship.
    SOLAS adopted the ISM Code in 1994 and incorporated it into chapter IX. By 1998 much of the commercial shipping community was required to be in compliance with the ISM code. By 2002 almost all of the international shipping community was required to comply with the ISM Code.
    In order to comply with the ISM Code, each ship class must have a working Safety Management System (SMS). Each SMS consists of the following elements:
     Commitment from top management
     A Top Tier Policy Manual
     A Procedures Manual that documents what is done on board the ship
     Procedures for conducting both internal and external audits to ensure the ship is doing what is documented in the Procedures Manual
     A Designated Person to serve as the link between the ships and shore staff
     A system for identifying where actual practices do not meet those that are documented and for implementing associated corrective action
     Regular management reviews"

    It should at least pay them, otherwise will not pay anyone.
    Nobody forced Schellino to graze the Giglio.
    He run away with the second and the third officer.
    He lied.
    It was on the bridge with an illegal.
    But my friend, what are we talking about?

    It should be put in jail, after having taken all he has.

    i'm surprised nobody commented on the ATB idea. It's a relatively modern invention, about 40 years old. So it's a proven system. The starship Enterprise on Next Generations could fictionally separate saucer from main hull.
    The best argument is the current liners could be retrofitted as ATBs. Expensive conversion but cheap compared to new construction.
    735 ft bulk ore freighters and some tank ships have already been converted to ATB.
    Nobody likes the idea? Lots of work for marine artchitechs! LOL :)

    What is the POSITION, responsibilty, of the officer behind the chart plotter and what is the position, responsibilty of the officer on lookout ? Who is piolting the ship ?

    Never having run a passenger liner, I don't know their procedure. But! It may be similar to on DP vessels (dynamic positioned for the few not familiar with DP). There are 3 officers on bridge each watch. One is on his feet observing and directing. Has the con. One is on the console with eyes glued to the screen. If he lifts his eyes, the con officer rebukes him. Third one is relaxing before going on console. They rotate every 20 minutes. Reason for this? It's too easy to be mezmerized by the electronic readouts, Go into a trance. Sort of asleep at the helm with eyes open.

    Generally speaking :

    Port position is for the officier of the watch.
    Starboard one is for the captain when present on the bridge.
    + one AB seaman minimum

    Special situations (restricted visibility, port/terminal entry, port pilot on board, etc) according to captain standing orders and bridge management procedure. Captain is normally on the bridge in such conditions, in addition to other officers and AB seamen (again according to ship's and company's procedures).

    The computer or auto-pilot is holding the course. If it is needed, the crew can take over at any moment.
