    This happens more then you realize. It's just a few that have a camera around when the crap hits the fan. This one shouldn't have happened and was pure operator error, as they had a preventer to keep the slings in place, but it is important to lower (and raise) the puppy reasonably level and any experienced lift operator knows this.

    How about the large tuna fishing boat that is innocently docked and then squashed like a soft drink can by a giant cargo ship whose captain is unable to stop? The boat has disappeared into bubbles by the time officials are on the scene:
