ok I,m thinking, usually injecter sleeves leak which either have holes or bad seats, leak overr to jacket
I,m not a Detroit expert, but on any make I can noy think how fuel can pass to Raw water, you sure its not oil? Cos oil can, from leaking cooler or many ways
In fact casting my mind back I,m sure its lube oil

You will have more oil pressure then water pressure when running.

fuel in exhaust water

this does appear to be fuel as when i check dip stick oil level has not changed ?
some has suggested a faulty injector spilling to much fuel into cylinder then being ejected with exhaust stroke ?

Would that be likely ?

does not matter, I have seen huge gears empty through leaking coolers I was FIELD SERVICEMAN fot Cummins Eng Co in early 1970,s before taking up boat and yachtbuilding, and I used to come across this sort of this quite often

Does the engine miss when running? You can pull the valve covers and short out one injector at a time and find the bad one.

John, You are making smoke right? black or white? both mean unburned fuel
which will show up in exhaust, check rack adjustment, injector height, also the injectors themselves, have they been o/hauled ,injector height needs special tool ......good luck
